Check is if the theme that is being used has its own version of this file. If you want your top_bar edit to be used in all themes, edit the tiki-top_bar.tpl file in the templates/ directory. But if a theme is chosen by a user or is set as the default them has its own tiki-top_bar.tpl file, such as in the templates/styles/moreneat/ (for example) directory, then that file will be used. Theme-specific template files have priority of the default template files, and editing the default file will have no effect for a theme with its own version of the file.
If you edited the default tiki-top_bar.tpl, and the theme you are using does not have its own version of this file, and the edit still hasn't taken effect, then be sure to clear your caches. Go to System Admin (tiki-admin_system.php) and click the "Empty" links under "Exterminator".
