There are two pieces to add:

1. Add the PHP code to the appropriate PHP file; in your case: tiki-view_forum.php
2. Add the HTML code to the appropriate TPL file; in your case: tiki-view_forum.tpl

FWIW, I've found http://www.protectwebform.com to be highly effective. My spam submissions and false registrations have been reduced to nearly 0.
