In Tiki 5, in the Look and Feel admin page, under the "Miscellaneous" tab, in the "Custom CSS" box, put
#tiki-top .wrapper, #middle {width: 100%;}
It's too late for Tiki 6, which is coming out soon, but probably in Tiki 7 (six months or so from now), Fivealive will probably respond to the Fixed-width admin checkbox similar to other themes (except that the header and footer backgrounds will always be full-width).
