Tacker comments and ratings

When setting up a tracker, the creator can allow ratings and comments and have them show when the item is listed. How do users rate the tracker items or add comments?

You can also play with the profile "Bug tracker" (or similar) from http://profiles.tikiwiki.org

And read the Documentation about Trackers (and help the Community improving the documentation, if possible)
